We have made it super easy to delete Docket® - Immunization Records account and/or app.

Things to note before removing Docket:

  1. The developer of Docket is Docket Health, Inc. and all inquiries must go to them.
  2. Check the Terms of Services and/or Privacy policy of Docket Health, Inc. to know if they support self-serve subscription cancellation:
  3. The GDPR gives EU and UK residents a "right to erasure" meaning that you can request app developers like Docket Health, Inc. to delete all your data it holds. Docket Health, Inc. must comply within 1 month.
  4. The CCPA lets American residents request that Docket Health, Inc. deletes your data or risk incurring a fine (upto $7,500 dollars).

Deleting from Smartphone 📱


Delete on iPhone:


  1. On your homescreen, Tap and hold Docket® - Immunization Records until it starts shaking.
  2. Once it starts to shake, you'll see an X Mark at the top of the app icon.
  3. Click on that X to delete the Docket® - Immunization Records app.

Delete on Android:


  1. Open your GooglePlay app and goto the menu.
  2. Click "My Apps and Games" » then "Installed".
  3. Choose Docket® - Immunization Records, » then click "Uninstall".

2. Idaho, New Jersey and Utah residents can access a SMART Health Card vaccine credential QR code to facilitate travel to places with COVID-19 restrictions.

3. NOTE: You must have a valid email address or phone number on file with your state's immunization registry in order to access personal or family immunization records using this app.

4. Please consult with your health department if you have trouble locating your records using this service.
