Deleting from Smartphone 📱
Delete on iPhone:
- On your homescreen, Tap and hold Concentra Telemed until it starts shaking.
- Once it starts to shake, you'll see an X Mark at the top of the app icon.
- Click on that X to delete the Concentra Telemed app.
Delete on Android:
- Open your GooglePlay app and goto the menu.
- Click "My Apps and Games" » then "Installed".
- Choose Concentra Telemed, » then click "Uninstall".

1. If you use Apple Health, you can choose to share your health information with the provider during your virtual video visit, such as heart rate, blood pressure, body temperature, blood glucose levels, weight, nutritional information, and respiratory rate.
2. Talk to an expert work health clinician or physical therapist using your smartphone or tablet for a video visit.
3. This app is intended for employees with authorization from their employer to evaluate and receive treatment for a work injury via remote provider, as available in areas where permitted.
4. • Once you’ve signed in, click on an available care coordinator to start your visit.
5. Use Concentra Telemed™ to get medical care and physical therapy right where you are, right when you need it.
6. • The care coordinator will check you in and ask some questions about your injury or illness.
7. • Using your email address, sign up for a Concentra Telemed account.
8. Before you start, make sure you have Wi-Fi or a strong signal.
9. • When they're done checking you in, you'll be connected with a clinician for treatment.
10. You'll also need a picture ID.
